Stealth Healthcare AI Startup

Software Engineer (Full-Stack)

ItalyEUR 60,000 - 80,000 on site

About the Role

Who we are

We're an ambitious startup building clinical AI software for the European healthcare market (and beyond). We use AI at its best - thoughtfully, responsibly and where it can make a meaningful difference - and give our team the best possible tools and equipment to create the greatest possible impact. We're looking for a Software Engineer who wants to own features end to end and help shape both the product and how we build it.

What you'll do

  • Design, build and ship features across the stack: backend services, APIs and user-facing interfaces.
  • Integrate AI components (LLMs, agents, pipelines) into a real product, with real constraints on correctness, latency and traceability.
  • Use agentic coding tools as part of your daily workflow, with the judgment to know when to trust them and when not to.
  • Contribute to architecture and product decisions: in a small team, your voice counts from day one.

What we're looking for

Requirements

  • 3+ years of professional software development experience.
  • Willingness to work full-stack, with genuine curiosity to pick up new tools, languages and technologies quickly.
  • Solid habits around automated testing, code review and CI/CD: quality as a practice, not a checkbox.
  • Interest in, or hands-on experience with, the professional use of agentic coding tools.
  • Autonomy and proactivity: we care deeply about the quality of how we work, and at this early stage not everything is defined yet. You'll play an active role in shaping the processes and practices we need as we grow.
  • Willingness to work in a regulated space: building health software comes with regulatory and compliance requirements, and handling them well is part of the engineering job.
  • Strong collaboration skills and empathy: you communicate openly, give and receive feedback well, and actively help keep the team culture healthy.
  • Effective written and spoken communication in English: it's our working language. Italian is a plus, not a requirement.

Nice to have

  • Knowledge of and experience with Rust.
  • Hands-on experience integrating and managing LLMs in a product, and making their output reliable and measurable.
  • Experience developing medical device software under MDR (Medical Device Regulation) certification.

Why join us

You'd be one of the first engineers on the team, working directly with the founders. The architectural decisions you make now will stay in the product for a long time.

Small team, fast decisions. Your impact is measurable from month one.

Full remote (EU, CET±2) or hybrid in Milan. We care about what you build, not where you sit.

Permanent contract and stock options: we want you to own a piece of what you're building.

You'll work on a real problem in a large, underserved and fast-growing market.

Compensation

RAL €60,000 - €80,000 gross annual salary, plus stock options.

We welcome applications from people of all genders, backgrounds and experiences. If this role excites you, but you don't tick every box, apply anyway: we'd rather hear from you.

This announcement is addressed to all genders (L. 903/77, D.Lgs. 198/2006).
